The Immigration Health Surcharge (IHS) is one of the biggest costs of a UK visa — so two questions come up again and again: can I get a refund? and where’s my IHS number? This 2026 guide answers both, with the current rates, when refunds are due and how to find your reference.

IHS in brief (2026 rates)

The IHS gives you access to the NHS and is paid upfront for the whole length of your visa:

  • £1,035 per year — most adult applicants
  • £776 per year — students and their dependants, Youth Mobility Scheme, and under-18s

IHS refunds: when you get money back

  • Visa refused or withdrawn: you get a full refund, usually automatically within about 6 weeks of the final decision.
  • Granted less time than you paid for: you get a partial refund of the difference.
  • Health and Care Worker visa: you and your dependants are exempt — if you paid, you’re refunded.

Otherwise the IHS is non-refundable (for example if you simply change your mind after the visa is granted). If a refund is due but doesn’t arrive, you can request it using your IHS reference.

Your IHS number: what it is and where to find it

When you pay the surcharge you’re given a unique IHS reference number (it starts with “IHS”). You’ll need it to:

  • complete your visa application;
  • register with an NHS GP once you’re here;
  • claim any refund.

Find it on the payment confirmation page and email you received when you paid. Keep it safe with your application documents.

Who is exempt from the IHS?

The main exemptions are Health and Care Worker visa holders and their dependants, and settlement (ILR) applications. Most other applicants staying longer than 6 months must pay.
